We are committed to customer service. Without our customers, we would all be unemployed. This Questionnaire attempts to measure your attitudes, values, and behaviors to help you and us better understand your strengths and possible training needs in light of our commitment to quality customer service. Your responses to this Questionnaire are confidential.
Directions:
- Please answer every question. Do not skip any. If none of the choices to a particular question describe your answer, select the one that most nearly describes you.
- You will have only one opportunity to answer each question. Do not hit your browser Back button as this will cause all your answers to be lost and you will have to start over.
- There are no "right" or "wrong" answers. The more honest your answers, the more you will likely benefit.
- Do not talk with anyone as you complete the Questionnaire.
- There is no time limit. However, do not take too much time on any one question.
- As much as possible, your answers should reflect what you actually do at work, not merely what you think, believe, or feel about any issue.
- When you have finished, be sure you go to Question 115 and click the Finish button. If you do
not click "Finish," your results will not be processed, and you will have to re-take the survey.
